Addition blepharoplasty

Fat melting is one of the significant phenomenon in the ageing of the face and is usually associated with tissue ptosis. Over the years, the eye surgery pioneers came to favour the techniques of lipofilling or restoring fatty volumes to restore the eyes in a natural and durable way.

Fat melting is generally most common in facial zones with significant muscular mobility where the repetition of movements and expressions are very frequent, such as eye blinking and smiling. This movement repetitiveness is responsible for faster and more significant fat reduction. This phenomenon is firstly and mainly visible around the eye socket, on the upper and lower eyelids.

A Word from Doctor Hayot

Today it is agreed by all eye surgeons that the principle cause of ageing of the periorbital area is the loss of volume linked to fat decreasing.

Classic blepharoplasty is a surgical technique that involves performing a skin and fat resection. Nevertheless, this surgical method has a tendency to age the eyes even more rather than rejuvenate them.

In light of this observation, a group of cosmetic experts moved away from the long-practised classic blepharoplasty in favour of a new cosmetic surgery technique: addition blepharoplasty.

Choosing upper addition blepharoplasty

Doctor Hayot changed attitudes towards practising blepharoplasty by transforming in the way people understood the rejuvenation of the eyes; a strategy whereby it is more effective to add volume rather than reduce it:

“During the first eye rejuvenation consultation, I systematically ask my patient to bring a photo of themselves when they were younger which enables me to analyse the process of ageing and to fulfil the requirements for natural rejuvenation, which are not clearly visible, as much as possible.

By comparing the photos of my patients when they were younger, I always notice a certain degree of loss of volume on the upper eyelid, especially from the inside and medial areas.

Filling blepharoplasty using lipofilling enables me to inject the patient’s own fat in order to create volume in the desired area (the upper and lower eyelids, nasolabial folds, cheekbones, etc.). The fat is removed from the knees or the abdomen using a small cannula, then, once collected, it is re-injected using a deep micro needle or cannula which reaches the bone of the patient. This surgical technique has been largely successful and the procedure is quick and safe.”

Advantages

When it comes to the lower eyelid, the loss of volume in the malar area accentuates the appearance of bags under the eyes. In effect, the malar area supports and maintains the lower eyelid and it is malar melting that leads to eyes looking tired and old.

When carrying out classic lower blepharoplasty, the cosmetic surgeon endeavours to remove the inside, medial and outside pockets by sometimes combining the method with muscular suspension (lower palpebral lifting) or with a skin resection.

However, the old method of classic blepharoplasty worsens the hollow at the junction between the eyelid and the cheek and makes it more visible, even if the bags under the eyes are removed

In order to avoid further hollowing the malar area, it is essential to choose addition blepharoplasty which involves injecting volume into the problem area rather than removing it, before removing the fat pockets in the same operating time.

Risks and side effects

Filling blepharoplasty using lipofilling is a quick and safe operation that takes place in an operating theatre under local or general anaesthetic.
In order to avoid taking any risks during the procedure, certain precautions must be taken:

  • Do not take anticoagulant medicines, such as aspirin, in order to reduce the risk of bleeding during the procedure.
  • Do not eat anything during the 8 hours before the procedure.
  • Get a prescription for the relevant medication from the cosmetic surgeon before the procedure (eyewashes, compresses, etc.).
  • Make sure your have cold compresses in order to reduce the postoperative oedemas.

Benefits of filing blepharoplasty for upper eyelids

Classic upper blepharoplasty involves removing the visible excess skin and an excision into the upper-inside pocket exacerbating the hollows of the upper eyelid. This surgical method results in the ageing of the eyelid by hollowing it rather than filling it to restore the “full” eyelid that the patient had in his or her youth.

What are results of addition blepharoplasty ?

Following lipofilling, the eyelids are swollen and oedematous. The postoperative treatments are not painful but the oedemas resulting from the eyelid operation can be considerable in the hours and days following the procedure.
The side effect of the swollen eyelids is completely normal and temporary. This effect disappears quickly. The patient does not feel any pain during or after the procedure.
